Apple Patch helps intellectually challenged and disabled individuals lead fulfilling lives. The Residential Direct Support Professional helps maintain the household for three residents, cooking meals, assisting participants in or doing light housework, administering medications, and at times providing personal care assistance for fragile or physically or behaviorally challenged individuals. Creatively encouraging and supporting residents to meet their own goals and to actively engage in their community is another fulfilling aspect of this participant-centered approach at Apple Patch. Second (4pm - midnight) and Third (Midnight - 8am) shifts available. This job will ask all of you in challenging ways but will give so much in return. -- In collaboration with the House Manager, develops and maintains a schedule of activities based upon the interest of the residents.
-- Assist clients in the development of daily living, social, recreational, and leisure skills.
-- Prepare or assist residents to prepare healthy meals following a menu.
-- Maintain cleanliness of the living unit and assists residents in their efforts to maintain the cleanliness of their individual rooms and common areas.
-- Assist with individual hygiene needs, including bathing, toileting, oral hygiene; etc. as needed.
-- Follow individual service plans for each individual client.
-- Maintain an electronic daily log of participant progress and activities.
-- Administer medications and document when complete in Medication Administration Record (MAR).
-- Offer basic first aid as needed.
-- Regularly attend mandated trainings and staff meetings.
-- Complete other documentation as required including incident reports and medication error reports.
-- Exhibits regular and punctual attendance.
-- Work collaboratively with other members of The Apple Patch staff.
-- Perform other duties as assigned.
